A probe by the Mumbai Metropolitan Region Development Authority (MMRDA) has found the Reliance Infrastructure-Astaldi joint venture responsible for the Metro Line 4 accident in Mulund that caused one death and injured three people. The report identified human error and lack of supervision as the main reasons behind the incident. It clarified that there was no flaw in design or materials. The authority imposed a penalty of INR 5 crore and initiated safety audits across Metro projects, highlighting concerns around execution practices and on-site monitoring.

The Mumbai Metropolitan Region Development Authority (MMRDA) has concluded that the contractor, a joint venture between Reliance Infrastructure and Astaldi, was responsible for the Metro Line 4 accident in Mulund that led to one fatality and injuries to three others.


The incident occurred when a precast parapet segment from an under-construction viaduct fell onto vehicles moving along LBS Road. The segment had been installed using temporary fasteners, but it collapsed after these supports were removed before the structure was fully secured.

According to the findings, a welder at the site cut the temporary supports even though the required stitching process had not been completed. The report indicated that this action was carried out prematurely and without proper clearance. It further stated that the lapse was not limited to one individual, as there was inadequate supervision by the site engineer and other responsible officials.

MMRDA clarified that the failure was not linked to any issue in design, engineering, or material quality. Instead, it pointed to negligence during execution and weak on-site monitoring as the key reasons behind the accident. The authority also noted that standard safety procedures were not properly followed during the installation process.

Following the findings, a penalty of INR 5 crore was imposed on the contractor. Additional fines were also levied on the project's general consultant for supervisory lapses. Authorities had earlier taken action against individuals involved in the construction and oversight of the project.

In terms of compensation, the family of the deceased received around INR 39.5 lakh. The injured individuals were provided financial support covering medical expenses of approximately INR 27.59 lakh.

The incident led to wider concerns regarding safety practices across ongoing Metro construction projects in Mumbai. As a result, MMRDA carried out detailed inspections across the entire Metro Line 4 corridor and extended safety audits to other under-construction Metro lines in the Mumbai Metropolitan Region.

Metro Line 4, which connects Wadala to Kasarvadavali, is a key infrastructure project aimed at improving connectivity in the eastern suburbs. The authority has since reviewed safety protocols and reinforced the need for stricter compliance at all construction sites to avoid similar incidents.

The contractor has not issued any public response to the report so far.
