Maharashtra to introduce five-star rating system for 623 government hospitals

Synopsis

The Maharashtra government has approved a five-star rating system for 623 public hospitals to evaluate and publicly display the quality of healthcare services across the state. Covering district, sub-district, rural, women's, general, trauma care and Employees' State Insurance hospitals, the initiative aims to improve service standards, increase transparency and strengthen accountability in government healthcare institutions. The ratings are expected to help patients make informed choices while encouraging hospitals to continuously enhance the quality of medical care.

The Maharashtra government has decided to introduce a five-star rating system for 623 hospitals functioning under the state Public Health Department as part of a broader effort to improve the quality, transparency and accountability of healthcare services. The initiative, announced through a Government Resolution (GR), seeks to establish a structured framework for evaluating public hospitals while making their performance information accessible to citizens.
The proposed rating mechanism will cover a wide range of government healthcare institutions, including district hospitals, sub-district hospitals, rural hospitals, women's hospitals, general hospitals, referral service hospitals, trauma care centres and Employees' State Insurance (ESI) hospitals. Together, these facilities constitute a significant portion of Maharashtra's public healthcare network and serve millions of patients each year across urban and rural regions.
The primary objective of the initiative is to create measurable benchmarks for assessing hospital performance and encourage continuous improvement in service delivery. By assigning star ratings based on predefined quality standards, the government aims to provide greater transparency regarding the level of care available at individual facilities while motivating hospitals to strengthen operational efficiency and patient services.
Public display of hospital ratings is expected to play an important role in enhancing accountability. Patients and their families will be able to view the quality assessment of healthcare institutions, enabling them to make more informed decisions when seeking treatment. At the same time, the rating framework is expected to foster healthy competition among government hospitals to improve their performance and maintain higher standards of patient care.
The evaluation process is likely to consider multiple aspects of hospital functioning, including infrastructure, cleanliness, patient safety, availability of medical equipment, quality of clinical services, administrative efficiency and overall patient experience. Although detailed evaluation parameters are expected to be implemented through the department's operational guidelines, the broader objective remains the systematic improvement of healthcare delivery across the public health system.
The initiative also aligns with the government's efforts to modernise public healthcare administration through performance-based monitoring. Structured assessment systems enable authorities to identify gaps in service delivery, allocate resources more effectively and prioritise improvements in facilities that require additional support. Regular evaluations can also assist policymakers in tracking progress over time and measuring the impact of investments in healthcare infrastructure.
For healthcare professionals and hospital administrators, the rating system is expected to encourage greater adherence to quality standards and operational best practices. Improved monitoring may also contribute to better maintenance of hospital facilities, enhanced patient satisfaction and more efficient utilisation of public resources.
The introduction of a five-star rating system reflects a broader shift towards outcome-oriented governance in the healthcare sector. By combining transparent public disclosure with structured performance evaluation, the Maharashtra government aims to strengthen public confidence in government-run hospitals while raising the overall quality of healthcare services delivered across the state. If implemented effectively, the initiative could serve as a model for quality assessment in public healthcare institutions across other states as well.
